I consent to the Anglo Medical Scheme administrator obtaining, accessing, processing and storing any information about me and sharing the information with entities that I have already given my consent to for the disclosure of this information. I also consent to the administrator group using the stored information, (1) in its depersonalised form for research and product development purposes; (2) to communicate essential information to me; and (3) to provide medical management and administration services to me. My consent is given on the understanding that, (1) my information will not be made available to any other third party without my express written consent; (2) the Anglo Medical Scheme administrator will conform to acceptable safeguards to ensure the privacy and confidentiality of my information; and (3) I may withdraw my consent at any time on notice to the Anglo Medical Scheme administrator.
