Member Brochure

Bankmed has introduced the Disease Prevention Programme to support its members who are at risk of diabetes or cardiovascular disease. If you meet the eligibility criteria and enroll in the Programme, you will receive 12 months of personalised support, including dedicated clinical care, health coaching, and access to additional Scheme benefits. These services are aimed at helping you improve your health and reduce your future health risks.

What is the Disease Prevention
Programme

Together with your Premier Plus GP, and with guidance from a Health Coach and a Dietitian in our network, this programme supports you in actively managing your risk of developing a chronic condition.

You'll receive additional benefits to help you monitor and manage your health, along with coordinated, high?quality care designed to deliver better health outcomes.

How to join the Disease Prevention Programme

We identify members who may be at risk of diabetes or cardiovascular disease by reviewing information such as your Personal Health Assessment results, health claims, family history, and other relevant data.

If you meet the Scheme's eligibility criteria:

  • A Health Coach will contact you to explain the Programme and how it can support your health.
  • With your consent, your Premier Plus GP can enroll you on the Disease Prevention Programme through HealthID

Benefits

Once enrolled, you have access to the following benefits:

Up to 2 consultations with your Premier Plus GP in our network, to support your ongoing health needs.

Up to 2 dietitian consultations within our network providing personalised nutrition advice to help optimise your health.

A defined set of blood tests, including up to 2 fasting blood glucose tests, 1 lipogram, and 1 serum creatinine test, to help monitor your health.

Diabetes medication (metformin), if prescribed by your Premier Plus GP and where clinically appropriate.

Up to 12 health coaching sessions, where our Health Coaches provide clinical support to help you track progress, monitor change and improve your overall health and quality of life.

Have you completed your Personal Health Assessment?

A Personal Health Assessment is a quick and valuable set of basic health screenings and preventive tests which you are covered for annually. It helps you understand your current health and identify potential risks early.

Your assessment includes:

  • Blood pressure screening
  • Blood sugar testing
  • Cholesterol or lipogram tests
  • Weight or body mass index (BMI) assessments
  • Non-smokers declaration (NSD)

Why should you do a Personal Health Assessment?

  • A Personal Health Assessment helps you take proactive control of your health by giving you valuable insights and support.
  • Understand your health status and, where applicable, the health of your family.
  • Identify potential health risks early, allowing for better treatment and management of more serious conditions, and helping determine your eligibility for the Disease Prevention Programme.
  • Reduce your healthcare costs over time by detecting and managing risks before they become more serious.
  • Support more personalised care, by giving your doctor and other Healthcare Professionals information to tailor treatment to your needs.
