If you meet the eligibility criteria and enroll in the Programme, you will receive 12 months of personalised support, including dedicated clinical care, health coaching, and access to additional Scheme benefits. How to join the Disease Prevention Programme
We identify members who may be at risk of diabetes or cardiovascular disease by reviewing information such as your Personal Health Assessment results, health claims, family history, and other relevant data.
If you meet the Scheme's eligibility criteria:
- A Health Coach will contact you to explain the Programme and how it can support your health.
- With your consent, your Premier Plus GP can enroll you on the Disease Prevention Programme through HealthID

Once enrolled, you have access to the following benefits:
Up to 2 consultations with your Premier Plus GP in our network, to support your ongoing health needs.
Up to 2 dietitian consultations within our network providing personalised nutrition advice to help optimise your health.
A defined set of blood tests, including up to 2 fasting blood glucose tests, 1 lipogram, and 1 serum creatinine test, to help monitor your health.
Diabetes medication (metformin), if prescribed by your Premier Plus GP and where clinically appropriate.
Up to 12 health coaching sessions, where our Health Coaches provide clinical support to help you track progress, monitor change and improve your overall health and quality of life.
Have you completed your Personal Health Assessment?
A Personal Health Assessment is a quick and valuable set of basic health screenings and preventive tests which you are covered for annually. It helps you understand your current health and identify potential risks early.
Your assessment includes:
- Blood pressure screening
- Blood sugar testing
- Cholesterol or lipogram tests
- Weight or body mass index (BMI) assessments
- Non-smokers declaration (NSD)

Frequently asked questions
The Programme uses an advanced predictive model to identify members who may be eligible. This is based on your Personal Health Assessment results. If your results show that you are at higher risk of developing diabetes or cardiovascular disease, you may be eligible for the Programme. Eligible members can then be enrolled by their Premier Plus GP.
The goal of the Disease Prevention Programme is to help reduce your risk of developing diabetes. This is done by identifying and addressing risk factors early, such as raised blood glucose levels. Support may include guidance on healthy eating, physical activity, and, where clinically appropriate, prescribed medication. During the Programme - and at its conclusion - your Premier Plus GP will work with you to determine the most appropriate next steps. This may include continuing lifestyle changes, ongoing medication, or further assessment. In some cases, a diagnosis of diabetes may be confirmed, allowing you to move forward with the right care and support
Your Premier Plus GP will help to register you for diabetes on the Chronic Illness Benefit (CIB), giving you access to benefits designed to help you manage your condition effectively.
